ID Session Registration

In consideration of my child’s participation in the Swish Summit Talent ID Session, I, the parent or guardian, hereby release, waive, discharge, and covenant not to take legal action against Swish Summit Basketball League, its officers, agents, employees, and volunteers from any and all liability for any injuries, losses, or damages to my child or property arising out of or in connection with the session activities. I understand that participation in basketball activities may involve inherent risks, including but not limited to injury, and I assume all risks associated with my child's participation.

I grant permission for Swish Summit Basketball League to take photographs and videos of my child during the Talent ID Session. I understand that these images may be used in promotional materials, including but not limited to brochures, websites, and social media. I waive any rights to compensation or ownership of these images.
